The signature of a parent or legal guardian will be required on or before the first day of participation. You may sign this form in person at the library before the event begins or when you arrive at the library with your teenager.
By signing this form, you give permission for the teenager listed above to participate in the Mathews Memorial Library Teen Summer Program. You agree to provide them a lunch each day and transportation to and from the library. Photos taken of participants during the program may be used on our website, social media or in-library displays promoting programming.
