Summer Youth Program Registration Form
I want to take part in The Adventure Begins. Please reserve a place for me at the events I have selected.
Registration on Tuesday, Thursday & Friday activities for participants age 6 -12 only. Registration on Wednesdays for participants age 5 and younger only.
I hereby grant permission to the Mathews Memorial Library to use my child’s photography on its website, social media, or other official printed publications.
The signature of a parent or legal guardian will be required on or before the first day of participation. You may sign this form in person at the library before the event begins or when you arrive at the library with your child.
By signing this form, you give permission for the child listed above to participate in the Mathews Memorial Library Summer Youth Program The Adventure Begins.
